Vaccination Registration
If you belong to the current vaccination phase, you can register for vaccination via NCZI application form.
Vaccination Registration As A Substitute
If you are a monitored patient with a chronic pulmonary disease, which is in the list of the Chief Hygienist, with a medical record from our outpatient clinic, you can register as a substitute for vaccination against COVID-19 on the website of the Bratislava Region. Such a patient will be registered as a substitute in a large-capacity Vaccination Center - Tehelné Pole (National Football Stadium), where it is currently in the use AstraZeneca vaccine. In the case of a free dose of vaccine, the patient must be able to arrive within 30-90 minutes.
If you are a patient with a chronic pulmonary disease, which is in the list of the Chief Hygienist, with a medical record from our outpatient clinic and you think, that AstraZeneca is not an appropriate option in your case for any reason, it is possible to apply for registration as a substitute for vaccination against COVID-19 with Moderna vaccine. As a substitute, a qualified patient can apply for registration at the Poliklinika Mlynská dolina, Poliklinika Mýtna, Železničná poliklinika a Ružinovská Poliklinika. In the case of a free dose of vaccine, the patient must be able to arrive within 30-90 minutes.
